The role of LRP5 in bone mineral density

Background. Loss-of-function variants in LRP5 cause low bone mass, while gain-of-function variants cause high bone mass. Whether common LRP5 variants matter in routine practice is less clear. We asked whether LRP5 genotype was associated with bone mineral density in adults referred for DXA.

Methods. Retrospective review of adults who had DXA and had consented to genotyping through a local research biobank. We compared lumbar spine and femoral neck BMD between carriers and non-carriers of the variant, adjusting for age, sex and body mass index.

Results. Carriers had lower adjusted BMD at both sites than non-carriers, and the difference was larger at the lumbar spine. The association held in a sensitivity analysis excluding patients on bone-active medication.

Conclusion. LRP5 genotype is associated with bone mineral density in a routine clinical population. A prospective study is planned to test whether genotype adds to fracture-risk prediction.

Reducing missed VTE risk assessments on a surgical ward: a two-cycle audit

Background. National guidance expects every admitted patient to have a VTE risk assessment on admission. A ward safety huddle raised concern that surgical admissions were being missed, particularly out of hours.

Methods. Cycle one reviewed consecutive emergency surgical admissions against the Trust standard. We then added a mandatory VTE prompt to the admission clerking proforma and covered it at doctors’ induction. Cycle two re-audited the same measure.

Results. Completion improved in cycle two. The assessments still missed were concentrated in weekend admissions clerked by locum doctors, who used an older proforma.

Conclusion. A proforma prompt is a cheap and effective fix. The old proforma has been withdrawn, and a third cycle will test whether weekend performance has caught up.

Near-peer simulation teaching for foundation doctors: a two-cycle evaluation

Background. Foundation doctors reported low confidence managing the acutely unwell patient out of hours. Formal simulation places were limited, so we piloted near-peer sessions led by core trainees.

Methods. Monthly one-hour scenarios in the ward simulation room, with structured debrief. Cycle one used pre- and post-session confidence ratings; after feedback, cycle two added a second scenario and a written debrief summary.

Results. Self-rated confidence rose after both cycles, with a larger rise in cycle two. Free-text feedback valued the debrief and asked for sessions to be protected from bleep cover.

Conclusion. Near-peer simulation is feasible without extra faculty and improves confidence. The Trust has adopted it into the foundation teaching programme.

What medical students value in ward-based teaching: a service evaluation

Background. Final-year students on their medicine placement described ward teaching as unpredictable. We wanted to know which kinds of teaching they found useful before changing the trainee teaching rota.

Methods. Anonymous online survey of final-year students at the end of the placement block, with rating questions and free text. Free-text answers were coded independently by both authors and themes agreed.

Results. Students valued bedside teaching with a named clinician, sessions that started on time, and being asked to present a patient. They valued least those sessions that repeated lecture content.

Conclusion. Students want predictable, patient-based teaching. The trainee rota now holds a protected weekly bedside slot, and a repeat survey is planned for the next cohort.
