Anaesthetics ST4 points calculator

Answer one question for each scored domain. We add up your points with the published rules shown on each step, then show what you could still add before the deadline, with dates. It takes about two minutes.

We'll show your result and email you a copy.

When are you applying?

We use this to date your plan. Applying this round, it works back from the deadline. Applying next year, you get the long-run version instead.

Your score is the total of the domain scores, out of a maximum of 50.

Maximum 6 pointsPostgraduate Additional Degrees and qualifications

1. Postgraduate Additional Degrees and qualifications

Scores the single best option you can evidence, up to 6 points. Pick the highest option you can evidence.

Postgraduate qualifications and non-anaesthetic postgraduate exams (MRCP, MRCEM, MRCS, DRCOG etc.). Anaesthesia/ICM exams (Primary or Final FRCA, EDAIC) and education qualifications do not score here.

Watch out. Claiming the Final FRCA or a PG Cert in medical education here: “The written component of the Final FRCA or complete FRCA does not score points” and education qualifications “are given credit in the teaching section”.

Caps. Single best option. Undergraduate degrees no longer score (removed from Round 2 August 2026).

Maximum 4 pointsAdditional Postgraduate Achievements, Prizes and Awards relative to medicine

2. Additional Postgraduate Achievements Prizes and Awards relative to medicine

Scores the single best option you can evidence, up to 4 points. Pick the highest option you can evidence.

Postgraduate prizes only; level set by the awarding organisation.

Watch out. Undergraduate prizes and patient thank-you letters: “Awards and Prizes in the Undergraduate period DO NOT COUNT” and “Letters of appreciation/thanks from patients DO NOT score points in this Domain.”

Caps. Single best option. Feb 2027 guidance: “3rd prize and lower placed prizes score 2 points from Regional and National Organisations.”

Maximum 4 pointsPostgraduate clinical experience in other specialties (excluding Foundation Programmes, Anaesthesia, research and teaching)

3. Postgraduate clinical experience in other specialties (excluding Foundation Programmes, Anaesthesia, research and teaching) • Up to 1 year of adult ICM can be included

Scores the single best option you can evidence, up to 4 points. Pick the highest option you can evidence.

Post-foundation clinical time in any non-anaesthetic specialty (including GP), counted to the time of appointment, with evidence of outcomes for every post.

Watch out. Assuming more is always better: more than 36 months scores 3, not 4. Missing evidence for any post caps you: “If applicants are unable to provide evidence for all posts undertaken, this will limit the score to 1 or 2 points”.

Caps. Max 4 is for 25–36 months; more than 36 months scores 3 (as printed in both the criteria grid and the Feb 2027 guidance). Adult ICU capped at 12 months.

Maximum 5 pointsPostgraduate Quality Improvement Projects (QuIP)

4. Postgraduate Quality Improvement Projects (QuIP) - can include audit as a tool to demonstrate quality improvement work in this section and trainee research network projects

Scores the single best option you can evidence, up to 5 points. Pick the highest option you can evidence.

Your role (participant, co-lead, lead) and when (since starting anaesthetics scores higher).

Watch out. Expecting extra points for presenting the audit: “Presentations of audit/QIP at local, regional or national meetings do NOT score additional points in this section but may be given points in Domain 8 if presented as a poster.”

Caps. Single best option.

Maximum 5 pointsResearch (excluding that completed during a PhD course)

5. Research (excluding that completed during a PhD course)

Scores the single best option you can evidence, up to 5 points. Pick the highest option you can evidence.

Depth of personal involvement in research, highest for leadership roles since starting anaesthetics.

Watch out. Using national audits or education research: “Involvement in large national audit projects e.g. NAP, NELA do not count in this section” and “Research related to postgraduate medical education is excluded in this domain”.

Caps. Single best option.

Maximum 6 pointsTeaching

6. Teaching

Scores the single best option you can evidence, up to 6 points. Pick the highest option you can evidence.

Delivered teaching (series, not one-offs), instructor roles, education posts and education qualifications.

Watch out. Expecting the course to add a point when you already teach: tier 1 is met by “some experience in formal teaching for health professionals” OR the course, so if you already score 1+ here the course adds nothing in Domain 6. Use it in Domain 9 instead, not both.

Caps. Single best option. A Teach the Teachers course reaches tier 1 only, and the same course cannot also be used in Domain 9.

Maximum 5 pointsAcademic publications

7. Academic publications

Scores the single best option you can evidence, up to 5 points. Pick the highest option you can evidence.

Peer-reviewed or PubMed-listed publications; a final acceptance letter counts. NMERC publishes optional full write-ups linked from each abstract. If peers review your write-up, NMERC can give you a letter saying it was reviewed by peers, as the additional evidence the guidance asks for when an article is not PubMed listed; the letter says so only when peers have actually reviewed it. The tier depends on what the write-up is, and the panel decides.

Watch out. Counting a poster abstract: “Publication of a poster abstract following presentation at a meeting does not score here.” Multiple e-BMJ case reports “count for a total of 1 point only”.

Caps. Single best option. No time limit: “we have removed the time limit of 5 years”.

Maximum 6 pointsPresentations and Poster Presentations

8. Presentations and Poster Presentations (see exclusions in applicant guidance)

Scores the single best option you can evidence, up to 6 points. Pick the highest option you can evidence.

Presentations actually given, ranked by setting (local → regional → national/international, defined by organising body) and timing (since starting anaesthetics scores higher for orals).

Watch out. Claiming a talk that has not happened yet, or a teaching session: “All posters and oral presentations must have been given by the time of document upload (not just accepted).” “Teaching presentations are excluded here (credit in Domain 6).”

Caps. Single best option. Level is set by the organising body, not the venue or audience.

Maximum 5 pointsContinuing Professional Development in Stage 1 Training or Equivalent (CT1-CT3)

9. Continuing Professional Development in Stage 1 Training or Equivalent (CT1- CT3)

Scores the single best option you can evidence, up to 5 points. Pick the highest option you can evidence.

Mandatory training plus attended 1-day (or longer) educational events within the last 5 years, with simulation/regional anaesthesia and national anaesthesia events needed for 4–5.

Watch out. Booked-but-not-attended courses and life-support/exam courses: “Events must have been attended and not just booked in the future.” “ALS, ILS … or any course required for foundation training does not score points.” “Exam courses (including revision courses) do not score points.”

Caps. Single best option. No mandatory-training evidence = 1 point off. Retained for Round 2 August 2027; removed from Round 2 August 2028.

Maximum 4 pointsActivities demonstrating leadership and/or management inside or outside of work

10. Activities demonstrating leadership and/ or management inside or outside of work. Leadership during school years will not be counted in this section.

Scores the single best option you can evidence, up to 4 points. Pick the highest option you can evidence.

Postgraduate leadership roles held long enough to show impact.

Watch out. New appointments: “Points for leadership/Management will be given if the applicant has been in the role for at least 3 months”. Teaching lead roles score in Domain 6, not here.

Caps. Single best option. Roles must have been held at least 3 months.

In the Anaesthetics ST4 self-assessment you can award yourself up to 50 marks across ten domains, and after an assessor has checked your evidence and added a Portfolio Organisation Score of up to 5, the verified score is out of 55. ANRO may use that score to shortlist, and it then makes up roughly a third of your overall score, with the interview carrying the other two thirds. For posts starting in August 2027, the expected application window runs from 19 November (10am) to 10 December 2026 (4pm), and you can’t edit your self-assessment on Oriel once you submit, so plan to have everything done by then (there is a route for later gains, explained below).

ANRO hasn’t published the August 2027 criteria yet. This page uses the grid for August 2026 and February 2027 starts, read alongside ANRO’s February 2027 applicant and assessor guidance from July 2026. Domain 9 (CPD) is due to go from August 2028, and ANRO says “This change is NOT implemented for the Round 2 August 2027/Round 3 February 2028 recruitment year.” Intensive care has its own separate grid, covered in our ICM ST3 self-assessment scoring guide.

What you can still add before 10 December 2026

What each of these adds is the difference between the option you would reach and the one you already hold, so it can be nothing if you hold that option already. The calculator works out the gain from your own answers.

  • Research (excluding that completed during a PhD course). Tier 2 only: start recruiting participants to a local TRN or NIHR portfolio study and get a supervisor letter confirming your role, ideally before you submit. Start by 19 November 2026. Expected national Round 2 opening. The Oriel self-assessment can’t be changed after you submit; a role confirmed later can still go to Qpercom with a request to change your score before the document upload window closes, and the assessor decides.
  • Teaching. If you currently score 0 here, attend a Teach the Teachers course (or deliver some formal teaching with evidence), ideally before you submit your application. Applications are expected to close at 4pm on 10 Dec 2026 (Round 2). Attend a live course date before you submit if you can; certificates are emailed after the course has run, so leave time for yours to arrive before you submit. If you attend after you submit, the February 2027 guidance lets you upload the certificate to Qpercom with a request to change your score, up to the close of the document upload window; the assessor decides.
  • Presentations and Poster Presentations. A national oral needs an accepted abstract, delivered by the time of document upload (before you submit, if you want it in your Oriel score). NMERC 2026 (online, Sat 14 Nov 2026; submit at nmerc.org/submit) is one option. Below 3 points: give one or two local/departmental presentations (not teaching sessions, not the audit already scored in Domain 4) and keep the dated slides. Expected national Round 2 opening; “All posters and oral presentations must have been given by the time of document upload (not just accepted)”. Presenting before you submit is simplest; a presentation given after you submit can be added on Qpercom with a request to change your score before the upload window closes, and the assessor decides.
  • Continuing Professional Development in Stage 1 Training or Equivalent (CT1-CT3). Attend another 1-day educational event (a Teach the Teachers course can count here instead of Domain 6), a simulation or regional anaesthesia day, or a course of more than one day, and keep the certificate plus programme. Attending NMERC (online, Sat 14 Nov 2026) can count as one 1-day educational event: “Any meeting, symposium, or conference relevant to anaesthesia or ICM may be credited here.” The panel decides. It is a separate event from the course, which still counts in Domain 6 or Domain 9, not both. Applications are expected to close at 4pm on 10 Dec 2026 (Round 2). Events must have been attended, not just booked: attend before you submit if you can and, for the Erudical course, finish the self-paced course too so both certificates can be uploaded. One attended after you submit can still be added on Qpercom with a request to change your score before the document upload window closes; the assessor decides.

Evidence traps

  • A Teach the Teachers course scores once: in Domain 6 (teaching) or Domain 9 (CPD), never both. If you already have any formal teaching experience, Domain 6 is already at 1, so put the course in Domain 9. Attendance at a "Teach the Teachers" course/Generic Instructors Course can count in either domain 6 or domain 9 but not both. source
  • An accepted abstract scores nothing until you have given the talk or poster. All posters and oral presentations must have been given by the time of document upload (not just accepted). source
  • For a regional, national or international presentation, slides alone are not enough: upload evidence of where you presented, such as the programme naming you as the speaker, the acceptance letter or a certificate describing the presentation. For Regional/National/International presentations, the applicant will need to show evidence of where the presentation occurred (not just the PowerPoint slides) source
  • Your self-assessment can’t be edited on Oriel once you submit. Evidence you gain after submitting, up to the close of the document upload window, can still go in: upload it to Qpercom with a request to change your score, and the assessor decides. Applicants who request a change of score due to additional evidence secured between application submission and the document upload window closing must submit this evidence on Qpercom source
  • Uploading more than 4 documents in a domain costs Portfolio Organisation Score points, so choose your best evidence. Applicants who present more than 4 documents will have their portfolio organisation score reduced by 1 point per domain (excluding Domain 3). source

Where you can still gain points before 10 December

For this round, five of the ten domains can’t realistically change: degrees, prizes, other-specialty experience, quality improvement and leadership, the last because a new role needs “at least 3 months” in post and one started now wouldn’t get there before the close. Publications rarely move in weeks, though an NMERC write-up reviewed by peers could (see below). The quickest gain is in Domain 9, where one more attended 1-day educational event, kept with its certificate and programme, moves mandatory training plus one event (2) up to 3, or mandatory training alone (1) up to 2, and a teach the teacher course can be that event, as can attending NMERC itself. In Domain 8, two local or departmental presentations since starting anaesthetics reach 3, and an oral you deliver at a national meeting such as NMERC (14 November) reaches 5, a gain of 1 over a regional poster or 2 over two local talks. If your research so far is data collection only, starting to recruit participants to a TRN or NIHR portfolio study before 19 November, with a supervisor’s letter confirming your role, lifts Domain 5 from 1 to 2.

Evidence that costs anaesthetics applicants marks

Evidence goes to Qpercom in a short window after applications close (last cycle it ran from 5 to 11 January 2026; the 2027 window hasn’t been announced), and a clinical assessor then verifies each domain. Your Portfolio Organisation Score starts at 5 and loses marks for messy or unlabelled domains, for more than four documents in any domain except Domain 3, and for a missing CV or training timeline in Domain 1, which every applicant has to upload whether or not they hold extra qualifications. ANRO strongly encourages a one- or two-page Domain Score Explanation for each domain, and that page isn’t counted as evidence. Two scoring rules surprise people: “The written component of the Final FRCA or complete FRCA does not score points” in Domain 1, and in Domain 3 more than 36 months of complementary experience scores 3, one less than 25 to 36 months.

ANRO’s applicant guide rules out editing your self-assessment score on Oriel after you submit. Its February 2027 guidance still leaves a way to count something you gain after submitting: “Applicants who request a change of score due to additional evidence secured between application submission and the document upload window closing must submit this evidence on Qpercom.” The assessor decides on each request, so finishing before you submit is the safer plan, and this is the fallback if a course or talk lands later.

Worked example: a CT3 anaesthetist on 19 marks

This trainee is in CT3 after ACCS, with no additional postgraduate qualifications, a Greatix award, a QI project they co-led in core training, some TRN recruitment, some formal teaching of medical students, one co-authored abstract, a regional poster, mandatory training plus one 1-day course, and a year as rota organiser. Domain by domain:

  • Domain 1, additional degrees: 0, “I have no additional degrees or qualifications.”
  • Domain 2, prizes: 2, as a Greatix award falls under “a prize or an award from my hospital or Trust since commencing anaesthetic training”.
  • Domain 3, other specialties: 2, because the 5 to 12 month tier “includes 6 months acute medicine and 6 months emergency medicine during ACCS training”.
  • Domain 4, quality improvement: 3 for co-leading, “a significant contribution to quality improvement work since commencing my core or stage 1 anaesthetic training”.
  • Domain 5, research: 2, “I have been involved with recruitment of participants to research projects (including TRN and NIHR CRN Portfolio studies).”
  • Domain 6, teaching: 1 for “some experience in formal teaching for health professionals”.
  • Domain 7, publications: 1 as “a co-author of (PubMed listed OR peer reviewed)” clinical abstract.
  • Domain 8, presentations: 4 for having “given a poster presentation at a regional, national or international meeting”.
  • Domain 9, CPD: 2 for “mandatory training/local online training plus one educational event/CPD event of 1 day duration”.
  • Domain 10, leadership: 2, “I can demonstrate leadership in management or leadership as a postgraduate in my hospital.”

That comes to 19 out of 50. Attending our teach the teacher course and claiming it in Domain 9 as a second 1-day event moves CPD from 2 to 3, so the total becomes 20, a gain of 1. Putting the same course in Domain 6 would gain nothing for this trainee, since their formal teaching already meets that 1-point tier, and the course can only be used once. If they also present an oral at NMERC in mid-November, Domain 8 moves from 4 to 5 under “I have given an oral presentation at a national or international meeting since commencing anaesthetic training”, another gain of 1 and a total of 21. A poster at the same meeting would add nothing, because their regional poster already scores 4. Both are best done before they submit; if either happens later, it goes in as a change-of-score request on Qpercom before the upload window closes. The Portfolio Organisation Score is assessed separately and isn’t included here.

The calculator scores your own answers against the same grid, one statement per domain, and works out how much each addition you have time for would add. We’ll show your result and email you a copy.

Does a teach the teacher course count for Anaesthetics ST4?

Yes, for 1 point in either Domain 6 or Domain 9. ANRO’s guidance says “Attendance at a ‘Teach the Teachers’ course/Generic Instructors Course can count in either domain 6 or domain 9 but not both.” In Domain 6 (Teaching) the 1-point tier includes “attended a Teach the Teachers course, Generic Instructors Course or similar”, but the same tier is met by “some experience in formal teaching for health professionals”, so Domain 6 is the better home only if you’ve never done any formal teaching. If you already teach, use Domain 9 (CPD), where the course counts as one educational event of 1 day duration: alongside your mandatory training it takes you from 1 to 2, or from 2 to 3 when it’s your second 1-day event. It can’t take you to 4 or 5 there, because those tiers need a simulation or regional anaesthesia event, an event lasting more than a day and, for 5, a national anaesthesia event. The course adds 0 if you already hold that tier or higher in the domain you claim it in. For Domain 9, upload both certificates, the live day’s (6 CPD hours) and the self-paced course’s (12 hours), together with the day’s 09:00 to 17:00 programme. The guidance says a certificate that doesn’t state the length of a course “MUST be accompanied by a course programme”, and it asks webinars to show “at least 7 CPD points”. Attend before you submit if you can (live dates before your deadline: 24 October, 31 October, 7 November, 15 November, 28 November and 5 December); a course attended later can still go in as a change-of-score request on Qpercom before the upload window closes. If you would rather teach on the course than attend it, Erudical is always on the lookout for tutors. NMERC and Erudical roles may qualify. See the NMERC and Erudical websites and contact us for details. For dates and booking, see the Teach the Teacher course for Anaesthetics ST4; how the same course fares in other specialties is set out in teaching points by specialty.

Giving your talk at NMERC before applications open

NMERC 2026 runs online on Saturday 14 November, which falls before the expected ST4 opening date. Doctors and medical students from any country may submit, presenting costs nothing, and a talk given there is already delivered when you score yourself on Oriel, which matters because “All posters and oral presentations must have been given by the time of document upload (not just accepted).” Once you’ve started anaesthetic training, a national oral scores 5 and orals at more than one national or international meeting score 6, so the gain is 5 if Domain 8 is empty now, 2 over two local talks and 1 over a regional poster. A poster at a regional, national or international meeting scores 4. Online meetings are judged the same way as in-person ones, and you need evidence you “presented in front of an audience of conference delegates (includes virtual conferences) as part of the conference programme”, so upload your slides with the programme naming you as presenter, or a certificate showing that you presented. A teaching session won’t score here: “Teaching presentations are excluded here (credit in Domain 6).” Whether you get a slot depends on your abstract being accepted. Full details of the meeting are on the NMERC page.

NMERC can help in two more domains. Attending the day can count in Domain 9 as one educational event of 1 day duration, since ANRO’s guidance says “Any meeting, symposium, or conference relevant to anaesthesia or ICM may be credited here”, and the panel decides whether NMERC meets that. It’s a separate event from the teach the teacher course, which still counts in Domain 6 or Domain 9 but not both, so the two together could be the two 1-day events that take you to 3; upload the day’s programme with your attendance evidence. In Domain 7, NMERC publishes optional full write-ups linked from each abstract. That domain accepts work that is “PubMed listed OR peer reviewed”, and “If an article is NOT PubMed listed, applicants must provide additional evidence demonstrating the publication is peer-reviewed”. If peers review your write-up, NMERC can give you a letter saying so, and it will only say that when peers have actually reviewed it. The tier depends on what the write-up is, and the panel decides; the abstract alone doesn’t score there, because “Publication of a poster abstract following presentation at a meeting does not score here.”

Common questions about ST4 anaesthetics scoring

How many points is a teach the teacher course worth for anaesthetics ST4?

One at most. It meets the 1-point tier in Domain 6, which formal teaching experience also meets, or it counts as one 1-day educational event in Domain 9, and it can’t be used in both. Choose the domain where it moves you up a tier; for most applicants who already teach, that’s Domain 9.

Does an accepted abstract count for the ST4 anaesthetics self-assessment?

No. The presentation has to have been given by the time of document upload. Presenting before you submit on Oriel is simplest; a talk given afterwards can still be added on Qpercom with a request to change your score, which the assessor decides. A national or international oral since starting anaesthetic training is worth 5 (6 for more than one meeting), and a poster at a regional, national or international meeting is worth 4.

What is the maximum ST4 anaesthetics self-assessment score?

50 across the ten domains: “Applicants can award themselves a maximum of 50 marks in unverified Self-Assessment.” The verified score is out of 55, including the 5-mark Portfolio Organisation Score. The interview is marked out of 100 with a pass mark of 60, and the self-assessment “contributes approximately one third of the total overall score”.

When do ST4 anaesthetics applications open for August 2027?

ANRO hasn’t posted its Round 2 2027 dates. NHS England’s national Round 2 timeline, which covers ST3 and ST4 jobs beginning from August to December 2027, gives a window of 19 November 2026 (opening 10am) to 10 December 2026 (closing 4pm). Interviews then run from 11 January to 9 April 2027, and the first offers are due by 5pm on 13 April 2027. Last cycle ANRO’s ST4 dates matched the national ones, so check the ANRO ST4 Round 2 page before you plan around them.
